If it is determined that you or your loved one is in need of a 24-hour skilled nursing facility after being admitted to the hospital, or within 30 days of discharge from the hospital, please have your physician or case manager send our admissions department all of your medical information for review. Acceptance is based on review of this information to ensure we can accommodate all of your medical needs, and on bed availability.
